Sensitive Information
I understand that some personal information is particularly sensitive, and that you are trusting me to keep this information confidential.
I will only collect sensitive information by methods that are reasonably secure, such as:
– Through an intake form in Acuity when you book an appointment such as a client questionnaire for kinesiology sessions. Client shared agreement with AFP or AMP.
– In a zoom consultation or face to face
– When you send me information in an email please note that email may not be sufficiently secure – if the information is extremely sensitive, ask me about alternative ways to share it with me.
The reason why I collect this information is:
– So that I can provide you with the services you have requested from me
– To ensure that I am providing you with the most appropriate services
The sensitive information I ask you to provide for this purpose may include:
– I collect on a client questionnaire form
– Your birth date
– Your medical history
– Information about your lifestyle and relationships
– Wellbeing and emotional status
I am committed to securely storing and handling your sensitive information.
STORAGE
Sensitive information is stored in a locked filing cabinet and on a password protected computer.
ACCESS
Only I the therapist responsible for your treatment and authorised team members may access sensitive material.
CLOUD STORAGE
I do not store sensitive information online or in the cloud.
Sensitive information may be collected from children under the age of 18 under the following circumstances:
– In the presence of their parents
– With their parent or guardian’s full consent
All information collected from minors is securely stored in accordance with this privacy policy.
DESTRUCTION POLICY
All archived sensitive information is securely destroyed after 7 years, children for 12 years, or 7 years after they turn 18.
You may choose not to provide me with your sensitive information. However,
If you choose not to be completely honest with me I may not be able to provide you with the services that you request. In your best interests my complete understanding (to the best of your awareness) helps me during consultation and in supporting you with the most appropriate recommendations.
As a Kinesiologist I am legally required to identify my clients by collecting their name and address, and failure to provide this information means I cannot offer my services to you.
Without complete transparency, my work with your cannot proceed as there will be an imbalance of the dynamic nature of our working together. In the field of energy medicine, openness and trust is the highest commodity.
Disclosure of Information
Reasons why I may disclose your information include:
– To provide you with the services you have requested
– To send you products that you have purchased
– When I use support services ( such as a VA)
In order to do this, I may share some relevant information – on a strictly need to know basis – with:
– My virtual assistant (VA)
– Australia Post or courier companies
– Third party providers who assist with: Auditing, Accounting or financial advice, Legal advice, Website team
LEGAL DISCLOSURE
I will also disclose your information if required by law to do so or in circumstances permitted by the Privacy Act – for example, where I have reasonable grounds to suspect that someone is engaging in unlawful activity, or misconduct of a serious nature, that relates to my functions or activities, and in response to a subpoena, discovery request or a court order. Also I have a duty of care to report cases of suicide ideation, suspected domestic abuse to the appropriate authorities.

DISCLOSURE OVERSEAS
I will use all reasonable means to protect the confidentiality of your information while in my possession or control. I will not knowingly share any of your information with any third party other than the service providers who assist me in providing the information and/or services I am providing to you. To the extent that I do share your information with a service provider, I would only do so if that party has agreed to comply with our privacy standards or I am satisfied that the service provider has a suitably protective policy of their own. Some of my service providers may be overseas and may not be subject to Australian Privacy Laws. Please contact me if you have any concerns about the potential disclosure of your information.
